To schedule an appointment or to obtain additional information about any of our mental health services, please fill out the form below or email to [email protected], with your preferred times below or give us a call. We will contact you to confirm available appointment times, a Visa or Mastercard must be provided to confirm an appointment. We will need you to have all the information from our patient registration form to confirm the appointment, you can email it to [email protected], fax it to (407) 903-9698, or we will take it over the phone when we call to confirm the appointment time.
If you are unable to make your scheduled appointment, Orlando Psychiatrist has a cancellation policy requesting 48 hours notice. Cancellations must be made by phone. Tel. (407) 903-9696. Services are available in-person in Orlando, and online throughout Florida.
If you're a new patient, please complete the following forms and bring them to your first session and please call your insurance company to find out your copayment, deductible and responsibility for mental health benefits.
We require you to fill out this form and send it to your previous doctors. This authorizes release of medical records to Bay Hill Psychiatric Associates. Fax all medical records to (407) 903-9698

The Psychiatric clinic is accepting new patients in Orlando, FL. Our board-certified child psychiatrist, Syeda Sultana, M.D., also treats adult psychiatry, and psychiatric evaluations for child/adolescent and adults.
